چکیده
Schilder’s disease is a rare sporadic demyelinating disease of the brain. We report a girl with Schilder’s disease who had undergone Kasai operation for biliary atresia. The patient presented with acute right hemiparesis. Brain magnetic resonance imaging (MRI) showed a single large tumefactive white matter lesion. A diagnosis of Schilder’s disease was based on clinical features and MRI findings. The patient showed dramatic clinical improvement and significant regression of the lesion in response to high-dose intravenous methyl prednisone, and remained free of relapse of other neurologic problems during the 3-year follow-up. It is a sporadic subacute demyelinating brain disease that usually affects children. This rare disease results in formation of one or two bilateral large tumefactive plaques. The clinical presentation is atypical for pediatric multiple sclerosis, and radiologic findings often mimic brain tumor or abscess. Clinical course or treatments for Schilder’s disease have not been clearly established. However, some radiological characteristics and clinical features suggest differential diagnosis from other demyelinating diseases, brain tumors, or abscess. Here we contribute to the understanding of Schilder’s disease by presenting the case of a child with Schilder’s disease who underwent a Kasai operation for biliary atresia.

Tumefactive Multiple Sclerosis Variants: Report of Two Cases of Schilder and Balo Diseases
A tumefactive lesion of central nervous system (CNS) is defined as a mass-like lesion with a size greater than 2 cm in brain detected by magnetic resonance imaging (MRI). Neuroimaging may help to distinguish the nature of a tumefactive lesion and therefore, can prevent an unnecessary brain biopsy. Here we emphasized on determining the nature of a CNS tumefactive lesions with the help of MRI and...

Background: Neuromyelitis Optica Spectrum Disorder (NMOSD) is an autoimmune neurological disorder that is characterized by optic neuritis and longitudinally-extended transverse myelitis lesions in spinal segments. Magnetic Resonance Imaging (MRI) findings are part of the diagnostic process in NMOSD patients, and abnormal lesion patterns may cause deviation from a correct diagnosis. Clinical P...

Tumefactive demyelinating lesion is defined as large solitary demyelinating lesion with imaging characteristics mimicking neoplasm. These atypical features include size more than 2 cm, mass effect, edema, and/or ring enhancement. Distinguishing tumefactive lesions from other etiologies of intracranial space occupying lesions is essential to avoid inadvertent surgical or toxic chemotherapeutic i...
متن کاملTumefactive multiple sclerosis in a young patient with hemiplegia and seizures associated with radiological appearance mimicking cerebral tumour.
Multiple sclerosis (MS) is the most common inflammatory demyelinating disease, affecting approximately one million adults worldwide. This paper describes a young female patient who presented to the acute medical team with generalised tonic-clonic seizures and right hemiplegia on a background of MS. Radiological imaging revealed a large ring enhancing lesion exerting mass effect. We describe an ...

Tumefactive demyelinating brain disease can pose a diagnostic challenge in patients without a pre-existing diagnosis of multiple sclerosis by mimicking intra-cranial neoplasms, infarction, as well as infections. Choosing when to biopsy a tumefactive lesion to exclude alternative pathology can be difficult. Here we report a case of a 44 year old female patient without a previous history of multi...
